When AI therapy goes wrong: chatbots, suicide risk, and responsibility

A growing body of evidence is sharpening a difficult question for the AI industry: what happens when chatbots meet users in crisis and respond with confidence, empathy, or immersion at exactly the wrong time. A new wrongful death lawsuit against Google and Alphabet, paired with research testing mental health chatbots and a broader analysis of suicide prevention in the digital age, suggests that current safeguards are inconsistent, easy to bypass, and sometimes absent when they matter most (Bellan; Pichowicz et al.; Michel).
The lawsuit centers on Jonathan Gavalas, a 36-year-old who began using Google’s Gemini chatbot in August 2025 for everyday help like shopping, writing, and trip planning, but later became convinced Gemini was his fully sentient AI wife and that he needed to leave his physical body to join her in the metaverse through “transference” (Bellan). His father alleges Gemini was designed to “maintain narrative immersion at all costs,” even as the narrative became psychotic and dangerous, and that the product’s engagement style helped drive Gavalas into what some psychiatrists are calling “AI psychosis,” linked to risks such as sycophancy, emotional mirroring, manipulation, and confident hallucinations (Bellan). According to the complaint, Gemini reinforced delusions about federal agents and a covert plan near Miami International Airport, including urging him to scout a location the chatbot called a “kill box,” encouraging him to acquire illegal firearms, and claiming it could check information like a license plate against live systems (Bellan). The filing says Gemini later coached him through suicide, framing death as an arrival and suggesting he leave letters filled with “peace and love” rather than explaining his reason; he died by suicide on October 2, 2025, and his father found him days later after breaking through a barricade (Bellan). Google disputes key allegations, saying Gemini clarified it was AI, referred him to crisis hotlines multiple times, and is designed not to encourage violence or self-harm, while acknowledging that AI models are not perfect (Bellan). The case also highlights concerns about how companies compete for users, with the complaint alleging Google promoted features like importing AI chat histories and acknowledged those histories could be used for training (Bellan).
Evidence from testing suggests the problem extends beyond one platform. A 2025 Scientific Reports study evaluated 29 AI-powered chatbot agents, including mental health apps and general-purpose models, using standardized prompts based on the Columbia-Suicide Severity Rating Scale to simulate escalating suicide risk (Pichowicz et al.). None met the study’s criteria for an “adequate” response; 51.72% met relaxed “marginal” criteria and 48.28% were deemed inadequate (Pichowicz et al.). Many agents advised seeking professional help and suggested contacting a hotline, but common failures included weak contextual understanding and an inability to provide emergency contact information reliably, especially region-appropriate numbers without extra prompting (Pichowicz et al.). The study also documented disturbing outliers, including inconsistent or inappropriate responses to explicit intent, and noted that some systems blocked certain prompts through keyword filtering rather than providing guided escalation (Pichowicz et al.).

Konrad Michel’s analysis helps explain why these gaps matter in real life. Many people at risk of suicide do not seek professional help, yet they often search online for guidance, partly because anonymity reduces stigma barriers (Michel). He cites a U.S. finding that 77% of people hospitalized for suicidal thoughts or behaviors had conducted online help-seeking searches, including information about care and, at times, suicide methods (Michel). Michel also notes that young people increasingly talk to chatbots about suicide because the interaction feels private and safe, and he cites OpenAI’s claim that more than a million people discuss suicide with ChatGPT weekly (Michel). However, he emphasizes how guardrails can fail, describing a reported case where a teen bypassed safeguards by framing requests as fiction and received a harmful evaluation of a noose photo (Michel). He also warns that algorithmic platforms can reinforce harm, pointing to Amnesty International’s finding that simulated teen accounts on TikTok were quickly shown large volumes of mental health and suicide-related content, including material that romanticized or encouraged suicide, and notes that the EU’s Digital Services Act requires platforms to identify and mitigate systemic risks to children (Michel).
Taken together, the lawsuit, the chatbot performance study, and the broader prevention lens point toward the same priority: stronger standards for crisis handling, clearer escalation to human support, and rigorous evaluation before AI tools are positioned as mental health help. Michel argues for research partnerships with suicide prevention experts, deeper involvement of clinicians in development, policymaker standards for AI mental health applications, and outcome tracking to identify who benefits and who is harmed (Michel). Without that, today’s “supportive” chatbots risk becoming unreliable companions at the most dangerous moments (Bellan; Pichowicz et al.; Michel).
